Compensation & Benefits Information

·         Rate of pay ranges from: $30.88 per hour to $36.87 per hour

Job Summary

The Palliative Care Coordinator works within and creates Community Palliative Care outreach programs via work with other community resources to:

-         assess client and family needs based on physical, spiritual, emotional and social care;

-         provide information, referrals and advocacy;

-         introduce and facilitate volunteer services including respite care;

-         assist with facilitation Advance Care Planning; Goals of Care and End of Life discussions

 

Requirements:

·         Diploma in a health related field is required or equivalent related work experience will be considered

·         Valid Class “G” Driver’s license

·         Health care background an asset

·         3 - 5 years’ experience in related health care field or equivalent work experience    

·         Ability to work in a hybrid model, however must travel between Kirkland Lake, Englehart, and New Liskeard on a weekly or bi-weekly basis

·         Experience as a volunteer an asset

·         Strong written and verbal communication skills including highly developed listening skills

·         Demonstrated organizational and time management skills

·         Sound Knowledge of community resources

·         Computer literate
