Brief description
The
overall goal of this position is to implement service coordination functions for
all Apitipi Anicinapek Nation children and their families, by providing them with
a knowledgeable resource to help them access health, education and social
supports through Jordan’s Principle; to contact Indigenous Services Canada on
behalf of such children and their families, and to navigate existing federal
and provincial/territorial health, educational programs and social services to
address a child’s needs. This position
will direct and oversee all youth services and requires outreach to families
and youth, as well as working collaboratively with Choose Life, Education,
Health, Social Development, Early Years, Apitipi Anicinapek Child & Family
Services and our Cultural Department of Apitipi Anicinapek Nation.
Following the Values of Apitipi Anicinapek Nation, we are seeking a dedicated and energetic individual who will work with our dynamic team to perform the following duties:
• Oversee and assist in the application for Jordan’s Principal funding on behalf of clients. Requests for application may come from individuals and applicable heath care service providers, community organizations.
• Provide a collaborative, client-driven system to guide children and their families through comprehensive array of services
• Support a child and family-centered approach to coordinating and connecting services
• Identify required services and supports that would benefit the child under Jordan’s Principle.
• Work towards establishing multi-disciplinary hubs for families when the services they require involves several professionals
• Help families navigate the health and social systems in a timely manner
• Assist with referrals for assessments to appropriate professional, or provide assessment if within the scope of practice
• Acquaint self with all Department of Indigenous Services Canada documentation related to the Canadian Human Rights Tribunal and Jordan’s Principle
• Act as a liaison and provide education and information sessions to community & staff in relation to Jordan’s Principle
• Work in collaboration and partnership with the Jordon’s Principal Writer to ensure that all individual, group and capital funding requests related to Jordan’s Principle are submitted.
• Participate in internal or external committees as required or requested
• Liaise and work effectively with the communities served, service providers, collateral agencies and organizations, elders
• Uphold confidentiality and safe keeping of all documents and records
• Develop and maintain work files that are accurate, up-to-date, and concise
• The ability to build relationships with families and community service agencies to effectively understand unmet needs of children and youth.

• Client file maintenance
• This job requires collection and reporting of data to the ISC
• Understanding of culture and holistic health models which support children
• Case management experience
• Any other duties as assigned
Required Qualifications
· A degree or diploma in any discipline or Minimum 2 years experience in any administration field
· Experience with organizing and coordinating community meetings.
· Experience with Microsoft 365, Excel, Word, PowerPoint, and Microsoft Teams
· Must be highly motivated, reliable, and committed.
· Effective oral and written communication skills
· Class G Driver’s License
· Criminal Record Check and Vulnerable Sector Check will be required.
Assets
· Valid First Aid/CPR
· Mental Health First Aid Certificate
· Crisis Intervention Training: ASSIST Training, Safe Talk Training, Peer Support Counselling
· Knowledge of both Indigenous and Non-Indigenous resources, services providers, groups, and programs that are in the district.
Lines of Communication
· The Jordan Principal Service Coordinator reports directly to Executive Director of Apitipi Anicinapek Nation
Working conditions
• This is a full-time position, and the working hours are from Monday to Friday, 7-hour days may include weekends maximum 35 hours a week. Additional hours may be necessary for the proper performance of the Employee’s duties.
